Job Description
Home Instead Senior Care in Hamden, CT, is looking for a Family & Community Liaison.
The Family & Community Liaison is expected to perform a variety of duties that relate to client referrals, client care consultations with potential clients and family members, client care including quality assurance visits with new and continuing clients, and client problem resolutions. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. They also perform duties related to researching and pursuing new referral sources and enhancing and maintaining relationships with existing referral sources. Finally, they communicate closely with other key players to ensure the availability of staffing new prospects who are likely to become clients.
Responsibilities
- Reflect the values of New Haven County Senior Care, LLC (d.b.a. an independently owned and operated Home Instead Senior Care franchise).
- Provide leadership and management of the client services efforts (client inquiries, care consultations, QA program) and participate in home care sales through targeted networking and increasing existing client service hours to meet changing client needs.
- Manage outcomes of Client Care activities (service inquiries, care consultations, QA program) and direct changes that will drive achievement of operational goals.
- Develop and maintain a structured, measurable and inclusive Quality Assurance program that builds trust with each client and CAREGiverSM.
- Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
- Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
- Develop and maintain positive and professional relationships with key referral sources in the community.
- Work with other staff towards a goal of high client satisfaction.
- Work with other staff towards a goal of high CAREGiver satisfaction.
- Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, referral providers and/or other care providers.
- Maintain regular attendance at the office to execute job responsibilities.
- Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.
Qualifications
- College degree.
- One year of experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
- Must possess a valid driver’s license
