Job Description

Demonstrates competency to assist with evaluating and carrying out nursing plan of care of geriatric adult, pediatric and adolescent patients per physician orders, policies, procedures and standards of care.

Essential Functions:

  • Provides age and culturally appropriate holistic patient care based upon facility mission statement, facility policies and the Nursing Division Standards, Policy and Procedures while ensuring adherence to standards of other regulatory agencies.
  • Implements interventions appropriate to the age specific needs of the patient requiring outpatient care.
  • Recognizes and anticipates common changes in the physiological, physical and health needs of all ages of patients experiencing urgent or non-emergent conditions.
  • Collaborates with RN to provide patient and family education designed to meet the specific needs of patient population served.
  • Provides safe interventions under supervision of RN ensuring that it is appropriate to patient's needs, age and condition.
  • Participates in hospital and employee health clinic activities to measure, assess and improve quality of nursing care delivered to patients.
  • Maintains confidentiality, security and integrity of data and information in accordance with HIPAA.
  • Facilitates communication via telephone between patient and medical staff.
  • Facilitates referral scheduling and forms completion as requested and approved by medical staff, i.e. insurance form and home health forms.
Additional Functions:

Other duties as assigned within scope and competency of the associate.

Requirements (Education, Experience):

  • Current LPN licensure with the Oklahoma State Board of Nursing or Compact License required.
  • Current BLS certification required.
  • Previous clinical experience required.
  • Must have the ability to push, pull and lift heavy objects weighing up to fifty (50) pounds.
